Job summary

Capital City Hospice in Columbus, OH seeks an experienced LPN/LVN to provide direct patient care within our hospice program. You will implement nursing plans, document services, and educate patients and families while coordinating interdisciplinary care through an on-site team.

The schedule is 3 twelve-hour shifts per week, with holiday on-call duties twice a year, supporting residents’ wishes to remain home. Competitive benefits are offered.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Current LPN/LVN license in the practicing state; CPR certification.
• Must have a licensed driver with insured vehicle per state requirements.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
• Coordinates hospice services, reporting needs to the interdisciplinary group.
• Provides appropriate pain/symptom management and evaluates response to treatments.

What You’ll Do:

• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical Supervisor
• Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
