Grace Hospice in Minneapolis, MN is seeking a compassionate Licensed Practical Nurse to join our interdisciplinary care team. As an LPN, you will provide hands-on nursing care focused on comfort, dignity and quality of life, working closely with the RN Case Manager to manage symptoms wherever patients call home.

You will educate families, respond to changing patient needs, and collaborate with a supportive team that values Excellence, Kindness and Teamwork.

Qualifications

• Minnesota LPN license is required.
• Graduate of an accredited practical nursing program.
• Hospice or home health experience is a plus.
• Strong assessment, communication and critical-thinking skills.

Responsibilities

• Provide direct nursing care per patient plan of care.
• Assess comfort and changing needs; communicate with RN Case Manager.
• Manage end-of-life symptoms and medications.
• Provide wound care, personal care and feeding support.
• Educate and support patients, families, caregivers, and facility partners.
• Collaborate with interdisciplinary team for coordinated care.
• Participate in bi-weekly IDG meetings.
• Complete documentation, follow safety and confidentiality practices.
