Job summary

Capital City Hospice is seeking an LPN to provide direct patient care in a hospice setting in Mt. Vernon, OH. The role focuses on comfort, symptom management, and quality of life, working with patients, families, and an interdisciplinary team.

The position offers a low patient caseload, supportive clinical oversight, and a work-life balance that fits a Monday–Friday schedule. An Ohio license and CPR certification are required, with prior hospice or home health experience preferred.

Qualifications

• Graduate of an accredited Practical Nursing or Vocational Nursing program.
• Current LPN license in the state of Ohio.
• Minimum one year of medical-surgical nursing experience.
• Previous hospice or home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation meeting organizational requirements.

Responsibilities

• Provide direct patient care in accordance with physician orders and established care plans.
• Educate patients and families regarding diagnoses, treatment plans, medications, and disease progression.
• Collaborate with the interdisciplinary hospice team to support comfort, symptom management and quality of life.
• Monitor patient conditions and communicate changes appropriately.
• Document patient visits, interventions and outcomes accurately and promptly.
• Support patients and families with compassionate, patient-centered care throughout the hospice journey.
