Job summary

CHI Health at Home Hospice is seeking a compassionate LPN/LVN to deliver direct patient care under approved treatment plans while following clinical protocols. You will collaborate with the RN Case Manager and health team to ensure adherence to the plan and guidelines.

Responsibilities include patient visits, documenting care, reporting changes promptly, educating patients and families, and maintaining accurate documentation.

Qualifications

• Graduate of an accredited practical nursing program and licensed to practice in the respective state.
• Current CPR or BLS certification and must be maintained during employment.
• Minimum of one year of nursing experience; home care or hospital nursing preferred.
• Excellent clinical skills and strong written and verbal communication abilities.
• Ability to work effectively within a multi-disciplinary team and build relationships.
• Strong organizational and time management skills.
• Valid driver’s license and compliant auto liability insurance as per policy.
• Experience with electronic health records, HomeCare HomeBase a plus.

Responsibilities

• Collaborate with the RN Case Manager and health team to ensure care per treatment plan.
• Conduct patient visits according to the physician’s plan, complying with regulations.
• Make skilled observations and document care accurately.
• Report changes in patient condition to supervisors and the physician promptly.
• Provide patient education to patients and caregivers on care plans and management.
• Demonstrate sensitivity to patients’ and families’ needs and culture.
• Participate in case conferences and in-services or orientations.
• Complete and submit documentation accurately and promptly.
