Job summary

UnitedHealth Group is hiring an LPN - Hospice to join the care team delivering compassionate home-based hospice services. You will verify plans, collaborate with physicians, teach self-care, monitor vitals, and complete timely documentation.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit
• Collaborate with physician and RN frequently to ensure patient needs are met
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review medications and perform medication reconciliation at each visit
• Perform specialized procedures ordered by physician (e.g. foley catheter change)
• Prepare equipment and materials for treatments using aseptic technique
• Complete documentation timely and accurately
