Job summary

CHI Health at Home Hospice seeks a compassionate LPN/LVN to deliver direct patient care in home settings per approved treatment plans. You will collaborate with an interdisciplinary team to ensure high-quality patient outcomes and adherence to clinical protocols.

Qualifications include graduation from an accredited practical nursing program, active state license, and CPR/BLS. A minimum of one year in nursing; home care or hospital experience preferred.

Qualifications

• Graduate of an accredited practical nursing program and licensed to practice in the respective state.
• Current CPR or BLS certification with hands-on demonstration is required.
• Minimum of one year of nursing experience; home care or hospital nursing experience preferred.
• Excellent clinical skills and strong written and verbal communication abilities.
• Ability to work effectively within a multi-disciplinary team and build interpersonal relationships.
• Strong organizational and time management skills.
• Valid Driver’s license and compliant auto liability insurance coverage as per organizational policy.
• Experience with electronic health records (EHR) systems, such as HomeCare HomeBase, is a plus.

Responsibilities

• Collaborate with the RN Case Manager and healthcare team to ensure effective patient care.
• Conduct patient visits per physician treatment plan while complying with regulations.
• Document observations of patients' conditions accurately.
• Report changes in patient condition to supervisors and physicians promptly.

Job Summary and Responsibilities

We are looking for a compassionate and skilled Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to join our team and help embody this mission. As an LPN you will deliver direct patient care based on approved treatment plans while adhering to clinical protocols and guidelines.

Responsibilities

• Collaborate and coordinate with the RN Case Manager and other healthcare team members to ensure effective patient care according to the treatment plan and patient care guidelines.
• Conduct patient visits as specified by the physician’s treatment plan, ensuring compliance with federal, state, and county regulations.
• Make skilled observations of patients' physical and mental conditions, documenting care provided accurately.
• Report any changes in patient condition to supervisors and the attending physician in a timely manner.
• Provide patient education to patients and caregivers, fostering understanding of care plans and health management.
• Demonstrate sensitivity to the needs, customs, and feelings of patients and their families.
• Participate in case conferences and in-services or special case orientations.
• Complete and submit all required documentation accurately and promptly.

Pay Range

$24.27 - $36.10 /hour
