Job summary

Qualifications

• Graduate of an approved school of Practical/Vocational nursing.
• Currently licensed to practice nursing in the state where the VITAS program is located.
• At least one year of nursing experience, including as an LPN/LVN in a hospital on an adult medical surgical unit.
• Equivalent experience may be considered.

Responsibilities

• Provides direct skilled bedside nursing care.
• Evaluates patient/family needs and assists in the interdisciplinary plan of care.
• Performs selected nursing acts under RN supervision, including administration of treatments and medications.
• Monitors treatment effectiveness and patient reactions.
• Reports significant findings to the RN, Team Manager or Physician and documents in notes.
• Maintains a clean, safe environment and accurate documentation on charts.
• Participates in developing and delivering interdisciplinary care.
• Establishes therapeutic relationships and involves patients/families in care planning.
• Initiates and performs nursing protocols to meet patient needs for comfort and well-being.
• Assists in patient/family education and discharge planning.
• Participates in ongoing professional growth and education activities.
• Cooperates with education and research activities as assigned.
• Participates in on-call rotation and may work weekends or rotating shifts.
