Job summary

VITAS Healthcare is seeking an LPN/LVN to provide terminal patient care under the plan of care in compliance with state Nurse Practice Act. The role emphasizes bedside nursing, patient assessment, and collaboration with the interdisciplinary team on the care plan.

The LPN/LVN will administer treatments and medications under RN supervision, document in clinical notes, and participate in on-call rotations with flexible weekend or shift schedules.

Qualifications

• Graduate of an approved Practical/Vocational nursing program.
• Current nursing license for the state where the program is located.
• At least 1 year of satisfactory nursing experience, including LPN/LVN in a hospital on an adult med/surg unit.
• Equivalent experience may be considered.

Responsibilities

• Provides direct skilled bedside nursing care.
• Evaluates patient/family needs and assists in the interdisciplinary care plan.
• Administers treatments and medications under RN supervision.
• Evaluates treatment effectiveness and monitors patient reactions.
• Documents findings in clinical notes and on charts.
• Maintains a clean and safe patient environment.
• Participates in on-call rotation and flexible scheduling.
