Job summary

VITAS Healthcare seeks a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) to provide direct bedside nursing care to terminally ill patients enrolled in the VITAS program. The role follows the plan of care and is conducted in accordance with state Nurse Practice Acts.

The LPN/LVN will evaluate patient and family needs, assist in interdisciplinary care planning, perform supervised nursing acts, document care, and maintain safe, clean environments.

Qualifications

• Graduate of an approved school of Practical/Vocational nursing.
• Currently licensed to practice nursing in the state where the VITAS program is located.
• At least one year of satisfactory nursing experience, including one year as an LPN/LVN in a hospital on an adult medical/surgical unit.
• Equivalent experience may be considered.

Responsibilities

• Provides direct skilled bedside nursing care.
• Evaluates patient/family needs and assists in planning of care.
• Performs nursing acts under RN supervision, including administration of treatments and medications.
• Evaluates effectiveness of treatments and monitors patient reactions.
• Reports significant findings to the RN/Team Manager or Physician and documents in clinical notes.
• Maintains a clean and safe environment and accurate documentation.
• Assists interdisciplinary team in designing and delivering the plan of care.
• Establishes therapeutic relationships with patients/families and involves them in care planning.
• Participates in on-call rotation and may work weekends or rotating shifts.
