Job summary

Anew Hospice in Lafayette, Indiana seeks an experienced LPN to provide direct patient care in the Lafayette and Kokomo areas. The role supports the plan of care under the RN and emphasizes compassionate, high-quality hospice services.

Responsibilities include implementing care plans, documenting services, and educating patients and families about diagnoses and self-care strategies. A strong teamwork ethic and HIPAA compliance are required to protect patient privacy.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three (3) years nursing experience.
• Community health/hospice or medical/surgical experience is preferred.

Responsibilities

• Provides direct patient/client care under supervision of RN.
• Implements plan of care and documents services accurately.
• Educates patient/family about diagnosis and self-care plan.
• Coordinates with interdisciplinary team to meet patient needs.
• Maintains aseptic technique and uses PPE as required.
• Assists with end-of-life care as per state nurse practice act.
