Job summary

Trilogy Health Services, LLC is seeking an LPN to provide direct clinical care and supervise nursing assistants for residents on assigned units in Kokomo, Indiana. The role emphasizes timely assessments, coordination with charge nurses, and adherence to care plans.

Responsibilities include daily resident visits, emergency response when needed, medication monitoring, and completing required documentation. A valid LPN license and CPR certification are required to ensure quality care and safety

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify appropriate staff.
• Identify resident problems and emergencies and initiate lifesaving measures in absence of a physician.
• Provide holistic care for residents within LPN scope.
• Monitor medication inventory and ensure adequate supplies.
• Perform administrative duties such as forms, reports, evaluations, and charting.
• Report occupational exposures per facility policies.
