Job summary

Paradigm Health in Indiana is seeking a licensed practical nurse to provide in-home hospice care, Monday through Friday, 8am to 5pm. The LPN will handle pain and symptom management, assess patient needs, and collaborate with the care team for optimal comfort.

Qualifications

• Indiana LPN license is required.
• CPR certification is required.

Responsibilities

• Provide skilled and palliative nursing in the home per plan of treatment.
• Evaluate clinical data and communicate status and needs to the team.
• Carry out pain and symptom management plans to keep the patient comfortable.
• Educate patient and family on pain management and notify RNCM if control is inadequate.
• Demonstrate knowledge of medications and proper administration based on patient age and condition.
