Job summary

UnitedHealth Group is seeking a Clinical Care Coordinator LPN for a full-time position based in Flagstaff, AZ. You will help manage patient care coordination, scheduling, referrals, and team collaboration to ensure continuous, high-quality end-of-life care.

The role emphasizes building strong relationships with patients and families. Qualified candidates hold an active LPN/LVN license, have a valid driver’s license, and are prepared to obtain CPR certification if not already certified.

Qualifications

• Current LPN/LVN license in state of practice.
• Valid driver’s license and access to a dependable vehicle or public transportation.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Helps manage order tracking, IDG preparation, and other patient care tasks.
• Monitors referrals and assigns RN/APN for start of care visits.
• Schedules visits to support continuity of care and relates with RN Case Manager/Social Worker.
• Participates in care team meetings to coordinate timely care and symptom management.
• Coordinates and tracks visits to comply with hospice policies and regulations.
