Job summary

Avanta Care Home Care - Atlanta is seeking a Hospice Licensed Practical Nurse (LPN) to work under the supervision of the RN Case Manager, providing direct nursing care, emotional support, and symptom management to hospice patients in home settings.

You will provide bedside care, assist with medications per plan of care, observe changes, perform wound and catheter care, document accurately, educate families, and participate in team meetings and quality improvement initiatives.

Qualifications

• Current LPN license (State of Georgia).
• Minimum 1 year of nursing experience, preferably in hospice or home health.
• Compassionate communication and team collaboration skills.
• Valid driver’s license and reliable vehicle.
