Job summary

Affinis Hospice in Newnan, GA is seeking a Licensed Practical Nurse to provide compassionate nursing care to hospice patients in accordance with the Plan of Care and state regulations.

The role requires current LPN license, CPR certification, and ability to work in home and facility settings. You will document patient status, educate families, and collaborate with the interdisciplinary team to promote comfort and dignity.

Qualifications

• Nursing degree from an accredited college or university or a graduate of an approved LPN program.
• Current license/registration by state of hire as an LPN.
• Current CPR certification.

Responsibilities

• Provide direct patient care as defined by the LPN scope and Hospice policies.
• Document patient services accurately and timely to reflect the Plan of Care.
• Educate patients, families and caregivers on diagnosis and Plan of Care.
• Collaborate with Interdisciplinary Group to coordinate services and report needs.
