Luminos Hospice in Missouri seeks a compassionate Hospice Licensed Practical Nurse (LPN) to join our team. The role involves direct patient care in home or facility settings, symptom management, and support for patients and families.

Responsibilities include comprehensive assessments, medication administration, and collaboration with an interdisciplinary team. Requires active Missouri LPN license and a minimum of one year nursing experience, preferably in hospice care.

Qualifications

  • Graduation from an accredited Licensed Practical Nurse program.
  • Current LPN license in the state of practice.
  • Minimum of one year of nursing experience, preferably in hospice.

Responsibilities

  • Provide direct patient care to hospice patients in homes or facilities.
  • Conduct comprehensive assessments of patient needs including physical, emotional, and spiritual aspects.
  • Administer medications, treatments, and interventions as prescribed by the team.
  • Monitor vital signs and overall condition, report changes promptly.
  • Collaborate with physicians, RNs, social workers, and chaplains.
  • Offer support to patients and families, ensuring compassionate care.
  • Document all care activities in the EMR.
  • Educate patients and families on symptom management and end-of-life care.
  • Coordinate with healthcare providers and community resources for continuity of care.
  • Participate in ongoing education to maintain licensure and skills.

  • Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment.
  • Document all patient care activities accurately and thoroughly in the electronic medical record.
  • Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care.
  • Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients.
  • Participate in ongoing education and training to maintain licensure and enhance clinical skills.
Requirements
Education

Graduation from an accredited Licensed Practical Nurse program.

Licensure

Current LPN license in the state of practice.

Current Driver's License.

Experience

Minimum of one year of nursing experience, preferably in hospice or palliative care.
