Job summary

Luminos Hospice in Overland Park, KS is seeking a dedicated Hospice LPN to provide compassionate nursing care in patients' homes or hospice facilities. You will assess needs, administer medications, and support patients and families through end-of-life care with a team approach.

Ideal candidates have an accredited LPN program, a current KS LPN license, at least 1 year in hospice or palliative care, and strong communication and organizational skills. Travel and reliable transportation required.

  • Communicate with the interdisciplinary team (physicians, RNs, social workers, chaplains).
  • Offer emotional support to patients and families.
  • Document care activities in the EMR.
  • Educate families on symptom management and care at end of life.
  • Collaborate with providers and community resources to ensure continuity of care.
  • Participate in ongoing education to maintain licensure and skills.

Job Summary:

The Hospice LPN provides compassionate and skilled nursing care to patients receiving hospice services. The LPN responds to patient needs, provides symptom management, and offers support to patients and families during end-of-life care.

Responsibilities:
  • Provide direct patient care to hospice patients in their homes or hospice facilities.

  • Administer medications, treatments, and interventions as prescribed by the hospice interdisciplinary team.
  • Monitor patient vital signs, symptoms, and overall condition, reporting any changes or concerns promptly.
  • Communicate effectively with the hospice interdisciplinary team, including physicians, registered nurses, social workers, and chaplains.
  • Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment.
  • Document all patient care activities accurately and thoroughly in the electronic medical record.
  • Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care.
  • Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients.
  • Participate in ongoing education and training to maintain licensure and enhance clinical skills.
Requirements
Education:

Graduation from an accredited Licensed Practical Nurse program.

Licensure:

Current LPN license in the state of practice.

Driver’s License:

Current Driver’s License.

Experience:

Minimum of one year of nursing experience, preferably in hospice or palliative care.

Skills:

Nursing skills as defined as generally accepted standards of practice.

Knowledge of state and federal regulations.

Ability to work independently and make sound decisions.

Strong attention to detail and organizational habits.

Excellent verbal and written communication skills.

Good interpersonal skills.

Transportation:

Reliable transportation and valid auto liability insurance.
