Qualifications

• LPN license in Idaho is required.
• Valid driver's license and auto insurance are required.
• Hospice experience preferred.

Responsibilities

• Provides direct patient care as defined in State's Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of hospice services, reporting needs for other disciplines to the RN or Clinical Supervisor/Director of Nursing.
• Uses equipment and supplies effectively and efficiently.
• Adheres to the organization's documentation and care procedures and standards of personal and professional conduct.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
