A healthcare service provider in Crystal Lake, IL is seeking a Hospice LPN for night shifts from 7PM-7AM. The ideal candidate will have substantial nursing experience, a valid LPN license, and CPR certification. HOURS: Monday - Friday (3) 12-Hour Shifts 7PM-7AM and Every Other Weekend

TERRITORY: Crystal Lake and surrounding area.

Salary: $26 - $38 hourly - Offer Based on Years of Experience

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill

What You’ll Do

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
