Job summary

Franciscan Alliance, Inc. is seeking an LPN to provide hospice home-based care in the Indianapolis area. You will implement plans of care, educate caregivers, and participate on the patient’s care team under RN supervision.

Responsibilities include assessing patient needs, documenting care, coordinating discharge planning, and collaborating with the multidisciplinary team to achieve improved outcomes. Travel is required up to 75% of the time.

Qualifications

• Must hold LPN license issued by the state licensing board.
• 1 year hospice experience preferred.
• 3 years acute or long-term care experience preferred.
• CPR certification required.
• Driver’s license required.
• Auto insurance required.

• Contribute to discharge planning and coordinate follow-up resources.
• Document patient care information and ensure timely records following policies.
• Educate patients and caregivers about hospice services and transition to ongoing care.

Licensed Practical Nurses who choose to focus on hospiceare a special group of people that can provide direct patient care, while, also, educating caregivers and actively participating on the patient’s care team. This is a level of nursing that must be able to accurately assess needs, provide appropriate interventions, and revise care as needed under the supervision of a registered nurse. WHAT YOU CAN EXPECT

• Implement nursing plan of care at the direction of the registered nurse.
• Provide home-based patient care based on physician orders and the established nursing plan of care.
• Report any change in patient status to the HospiceRN and document, as appropriate.
• Collaborate with HospiceRN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.
• Conduct patient psychosocial assessments, to explore the physical, psychological and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs in order to inform care team.
• Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
• Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
• Participate in the education of patients and their caregivers about extended care facilities, hospiceagencies, and other resources, as appropriate to transition patients along the continuum of care.

QUALIFICATIONS

• Associate’s Degree OR Diploma from Accredited LPN Program- Required
• 1 Year Hospice Experience- Preferred
• 3 Years Acute or Long Term Care Experience- Preferred
• Licensed Practical Nurse (LPN)- State Licensing Board- Required
• Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
• Driver’s License- Required
• Auto Insurance- Required
