Job summary

CHI Franciscan Health at Home seeks a compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver direct patient care at home under approved treatment plans. Join a multidisciplinary team, collaborate with the RN Case Manager, perform patient visits, document care, educate patients and caregivers, and ensure regulatory compliance.

Requirements: graduation from an accredited practical nursing program, active state licensure, current CPR/BLS, and a minimum of one

Qualifications

• Graduate of an accredited practical nursing program and licensed in the respective state.
• Current CPR or BLS certification with hands-on demonstration.
• Minimum of one year of nursing experience; home care or hospital nursing experience preferred.
• Excellent clinical skills and strong written and verbal communication abilities.
• Valid Driver’s license and compliant auto liability insurance coverage as per organizational policy.

Responsibilities

• Collaborate with the RN Case Manager and healthcare team to ensure effective patient care.
• Conduct patient visits as specified by the physician’s treatment plan, ensuring compliance with regulations.
• Make skilled observations of patients' physical and mental conditions, documenting care provided accurately.
• Report any changes in patient condition to supervisors and the attending physician in a timely manner.
• Provide patient education to patients and caregivers, fostering understanding of care plans and health management.
• Demonstrate sensitivity to the needs, customs, and feelings of patients and their families.
• Participate in case conferences and in-services or special case orientations.
• Complete and submit all required documentation accurately and promptly.

As an LPN you will deliver direct patient care based on approved treatment plans while adhering to clinical protocols and guidelines.

• Collaborate and coordinate with the RN Case Manager and other healthcare team members to ensure effective patient care according to the treatment plan and patient care guidelines.


• Conduct patient visits as specified by the physician’s treatment plan, ensuring compliance with federal, state, and county regulations.


• Make skilled observations of patients' physical and mental conditions, documenting care provided accurately.


• Report any changes in patient condition to supervisors and the attending physician in a timely manner.


• Provide patient education to patients and caregivers, fostering understanding of care plans and health management.


• Demonstrate sensitivity to the needs, customs, and feelings of patients and their families.


• Participate in case conferences and in-services or special case orientations.


• Complete and submit all required documentation accurately and promptly.
