Job summary

Ohio’s Hospice seeks a Hospice Individualized Care LPN for day shifts (8am–8pm) in the Dayton area. The role involves 1:1 bedside care for patients who are actively declining, traveling to patients in homes or facilities under RN supervision.

Must hold an Ohio LPN license, CPR, and a valid driver’s license with insurance; one year of nursing experience preferred and ability to document care. We offer competitive pay, mileage reimbursement, and ongoing education opportunities.

Qualifications

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR Certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen

Responsibilities

• This position will provide 1:1 with the patient at the bedside for patients that are actively declining.
• As a Hospice Individualized LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.
