Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice LPN to deliver patient-centered care in Palm Beach County. Work hours 8:30am–5pm with weekend availability as needed.

The LPN will assess needs, develop care plans, provide teaching and support, and coordinate with RN and physicians to ensure comfort and dignity for patients in hospice care.

Qualifications

• Current LPN license in Florida and active BLS certification.
• 30-hour IV course is a plus and may be required by some facilities.
• Reliable transportation with valid Florida driver’s license.

Responsibilities

• Assess patient and family needs under RN and physician supervision.
• Coordinate and implement individualized care plans emphasizing comfort and dignity.
• Utilize monitoring, intervention, and teaching skills to improve quality of life.
• Provide emotional support to patients and families during overnight care.
• Ensure personalized service and a positive patient experience.

Job description

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Palm Beach County area. You will work 8:30am - 5pm with weekend availability.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You’ll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
