Job summary

Residential Home Health and Residential Hospice is seeking an LPN to provide end-of-life hospice care in patients' homes around Lewisburg, PA. The role offers autonomy with strong interdisciplinary teamwork, focus on comfort and quality of life, and opportunities for professional growth.

You will travel to patient homes within the assigned territory, conduct symptom management, educate families, and document thoroughly in EMR.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work.
• Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred.
• Due to the field-based nature of this home hospice position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Conduct ongoing patient assessments focused on comfort, symptom management, and changes in condition.
