Job summary

Interim HealthCare of Augusta is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to provide patient-centered end-of-life care in Greenville and Spartanburg counties. You will combine clinical excellence with empathy, supporting patients and families through a meaningful transition.

You'll work with an experienced team, deliver timely documentation, administer medications, educate families, and ensure comfort.

Qualifications

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Active license to practice as LPN/LVN in the state where hospice care is provided.
• Minimum one year of nursing experience.
• Hospice experience preferred.

Responsibilities

• Frequent and timely communication to the appropriate RN, supervisor and/or other staff.
• Document patient services to reflect the plan of care accurately and promptly.
• Educate patients and families about diagnosis and self-care.
• Administer medications and treatments as prescribed by the physician and nursing standards.
• Coordinate hospice services and report identified needs to the hospice interdisciplinary team.
• Ensure availability of equipment, supplies, drugs, and other necessary items and services.
• Provide education and interventions to manage patient pain and other physical symptoms.

WhatYou’llDo

As a Hospice Licensed Practical Nurse (LPN), you will:

• Responsible for frequent and timely communication to the appropriate registered nurse, supervisor and/or other staff members regarding the patient’s condition. Communication includes participating in case conferences and other more immediate methods of communication when indicated.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
• Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.
• Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members.
• Ensures that equipment, supplies, drugs, biologicals and other necessary items and services are available.
• Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment.
