Job summary

The Care Team Hospice seeks a Licensed Practical Nurse (LPN) to deliver compassionate, hands-on nursing care in home and facility settings around Mishawaka, IN. You will work under an RN and team with the interdisciplinary group to ensure comfort and dignified end-of-life care for patients and families.

Standard hours are Monday–Friday, 8:00 AM–4:30 PM, with up to a 1-hour radius from Mishawaka. We offer a competitive benefits package including 401(k) with employer match, HSA with employer

Qualifications

• Active LPN license in good standing.
• Driver's license required with clean driving record.
• CPR certification as required for the role.
• Ability to pass background and MVR checks.

Responsibilities

• Provide nursing care as delegated by the RN.
• Monitor patient status and report changes promptly.
• Support comfort and symptom management for patients.
• Document care provided in the EMR accurately.
• Collaborate with the interdisciplinary team to deliver holistic care.
