Job summary

Avanta Care Home Health is seeking a Hospice Licensed Practical Nurse (LPN) to work under the supervision of the RN Case Manager. The LPN will provide bedside nursing, symptom management, and emotional support to hospice patients in their homes.

Responsibilities include medication administration per plan, observation and reporting of patient changes to the RN or physician, wound and catheter care, documentation, and family education. A valid GA LPN license and a reliable vehicle are required.

Qualifications

• Must hold a Georgia LPN license and be in good standing.
• Minimum 1 year of nursing experience, preferably in hospice or home health.
• Valid driver’s license and reliable transportation.
