Job summary

Concordia Hospice of Washington is seeking a Licensed Practical Nurse (LPN) to join our hospice team in the Avella/Washington area. You will provide skilled nursing care, administer medications, and educate patients and families to support comfort and dignity at end of life.

In this role, you will work autonomously within a collaborative team, manage patient care plans, and maintain medical records while delivering compassionate support to patients and families during challenging times.

Qualifications

• Must hold a current LPN license.
• Experience in home health or hospice preferred.
• Ability to work independently and as part of a team.
• Strong communication and compassion.

What you’ll do as a Licensed Practical Nurse LPN:

As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Administering medications and treatments
• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records

The roots of our organization can be traced back to Rev. Dr. J. E. Victor Carlson, who helped create Hospice Care Inc. in Greene County in the late 1970s to serve families dealing with end-of-life illnesses. In 1996, the program was combined with The Washington Hospital, opening opportunities for inpatient care and other new services. On July 1, 2016, Concordia Community Support Services and Washington Health System (previously The Washington Hospital) partnered to form the organization we are today.
