Job summary

AdvisaCare Hospice in Michigan is seeking an experienced Licensed Practical Nurse (LPN) for a 1-on-1 hospice care role. The LPN will provide compassionate end-of-life care, participate in interdisciplinary plans, and help patients live their final days with dignity.

The position emphasizes one-on-one patient interaction, field visits, and on-call availability to support families and caregivers in the Grand Rapids area. Apply to join a dedicated hospice team.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily, including on-call scheduling.

Responsibilities

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Medicare requirements.
• Educate patient and family on disease process, self-care techniques, and end-of-life care.
• Supervise paraprofessionals providing services to patient.
• Prepare clinical and progress notes, participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families, and caregivers.
• Communicate information using current process and technology within the organization.

Are you looking for a challenging and rewarding position as a Hospice LPN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Community with our Troy office location being your Parent Agency servicing our clients for the Grand Rapids and surrounding areas!

For this role, you need to be a Licensed Practical Nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

Requirements:

• Current Michigan License in good standing

• Ability to adapt to patient’s status and needs on a full time on call basis
