Job summary

AdvisaCare is seeking a Full Time Hospice LPN to join our compassionate team serving the Tri-Cities and surrounding areas in Michigan. You will provide critical hospice nursing care, support patients and families, and participate in field visits with a on-call schedule as needed.

The ideal candidate holds a Michigan LPN license in good standing and has a compassionate, organized approach. Hospice experience is preferred but not required; we offer flexible scheduling and a supportive work

Qualifications

• Current Michigan LPN License in good standing.
• 1 year hospice experience preferred but not required.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily and attend weekly office meetings as indicated.
• Ability to adapt to patient status and needs on an on-call basis.

Responsibilities

• Provide compassionate hospice nursing care to patients in home or facility settings.
• Assess patient conditions and adjust care plans accordingly.
• Collaborate with families and the care team to support patient comfort and needs.
• Participate in field visits and on-call coverage as required.
