Job summary

Good Samaritan Hospice | Concordia Lutheran Ministries is seeking two compassionate RN Liaisons to join our team. You will meet with patients and families, perform clinical assessments, and guide them through hospice decisions with empathy and clarity.

In this role you will cultivate partnerships with hospitals and referral sources, coordinating smooth transitions into hospice services while supporting admissions as needed and ensuring high-quality end-of-life care.

Qualifications

• Current Registered Nurse (RN) license in the state of Pennsylvania
• Minimum of two years nursing experience
• Marketing/Public Relations experience preferred
• Admission and discharge planning experience preferred
• Knowledge of hospice and palliative nursing preferred

Responsibilities

Are you a compassionate RN who enjoys connecting with people and making a meaningful impact? Good Samaritan Hospice is looking for two full-time Hospice RN Liaisons to join our team!

In this role, you’ll meet with patients and families who are considering hospice care—providing clinical assessments, answering questions, and helping them navigate an important decision with confidence and comfort. You’ll also build strong relationships with hospitals, referral sources, and community providers to ensure a smooth and supportive transition into hospice services.

This is a great opportunity for a nurse who enjoys both clinical care and relationship-building. You’ll also assist with admissions when needed and play a key role in ensuring patients receive compassionate, high-quality end-of-life care.

QUALIFICATIONS FOR HOSPICE NURSE LIAISON
