Job summary

Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, seeks a compassionate LPN to provide quality hospice care in our 6-bed Inpatient Unit located in Wexford, PA. This full-time role involves three 12-hour shifts per week with rotating daylight and overnight shifts, weekends, and holidays.

The LPN will assess patients, administer care, support families, and collaborate with an interdisciplinary team to implement individualized plans of care in a peaceful hospice environment.

Qualifications

• Licensed Practical Nurse (LPN) license in Pennsylvania.
• Minimum of two years of patient care experience preferred.
• Strong clinical and assessment skills are required.
• Excellent communication and interpersonal skills.
• Strong organizational skills with attention to detail.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Performs nursing assessments and monitors the patient's condition during each shift, reporting changes to the hospice team.
• Provides skilled nursing and personal care including medication administration, vital signs, feeding, ambulation, and hygiene.
• Gives compassionate physical, emotional, and spiritual support to patients and families with the hospice team.
• Provides care at the time of death following hospice procedures and guidelines.
• Completes all required documentation accurately and on time.
• Works a rotating schedule including daylight and overnight shifts, weekends, and holidays.

Job description

Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality hospice care to patients and support to their families in our 6-bed Inpatient Unit located in Wexford, PA.

This is a full-time position working three (3) 12-hour shifts per week. The schedule includes rotating daylight and overnight shifts, weekends, and holidays.

POSITION SUMMARY

The Licensed Practical Nurse provides skilled nursing and compassionate end-of-life care to hospice patients in the inpatient setting. The LPN works collaboratively with the patient, family, physician, and hospice interdisciplinary team to provide care in accordance with the patient's individualized plan of care.

The LPN plays an important role in creating a peaceful, supportive environment for patients and families during an important and often difficult time in their lives.

QUALIFICATIONS

RESPONSIBILITIES

1. Performs nursing assessments and monitors the patient's condition throughout each shift, reporting changes in condition to the appropriate members of the hospice team.
2. Provides skilled nursing care and personal care, including, but not limited to:
   • Medication administration and monitoring
   • Monitoring vital signs and patient symptoms
   • Bathing, grooming, dressing, and shaving
   • Feeding and assisting with meals
   • Assisting with ambulation and transfers
   • Repositioning patients and changing bed linens
   • Assisting with elimination
   • Providing oral, skin, foot, and ear care
   • Light housekeeping and meal preparation as needed
3. Provides compassionate physical, emotional, and spiritual support to patients and families in collaboration with the hospice team and individualized plan of care.
4. Provides appropriate support and care to patients and families at the time of death, following hospice procedures and guidelines.
5. Completes all required electronic and/or written documentation accurately and within established timeframes.
6. Works a rotating schedule that includes daylight and overnight shifts, weekends, and holidays as required.
