Hospice Liaison / Care Coordinator
Job Status: Full-Time
Part-Time and Per Diem candidates may also be considered based on experience, availability, and organizational needs.
Position Summary
The Liaison / Care Coordinator is responsible for developing and maintaining professional relationships within the healthcare community while helping connect patients, families, facilities, and referral partners with appropriate hospice services.
This position combines community outreach, relationship development, referral coordination, education, and follow-up. The Liaison / Care Coordinator works closely with the hospice team to ensure referrals and community partners receive timely, professional, compassionate, and personalized support.
Previous healthcare industry experience is required.

Essential Duties and Responsibilities
- Develop and maintain professional relationships with healthcare providers, assisted living communities, independent living communities, group homes, skilled nursing facilities, hospitals, physician offices, home health agencies, senior-care professionals, and other community partners.
- Educate referral sources and community partners regarding hospice services, eligibility, available resources, and support available to patients and families.
- Identify opportunities to develop new referral relationships while strengthening and maintaining existing partnerships.
- Conduct regular in-person visits, follow-up calls, emails, networking activities, and community outreach.
- Serve as a professional point of contact for referral partners and assist with communication between referral sources and the hospice team.
- Assist with potential referrals and promptly communicate referral information to the appropriate clinical or intake staff.
- Follow up appropriately with referral sources while maintaining patient confidentiality.
- Coordinate or participate in meetings, presentations, educational opportunities, community events, facility activities, and relationship-building opportunities.
- Maintain accurate records of outreach activities, contacts, referral activity, follow-up needs, and relationship development.
- Communicate referral-source needs, community feedback, potential opportunities, and concerns to leadership.
- Collaborate with clinical, intake, social work, chaplain, administrative, and other team members as needed.
- Maintain current knowledge of hospice services, programs, service area, and available resources.
- Represent the organization professionally, compassionately, ethically, and respectfully within the community.
- Maintain HIPAA compliance and protect confidential patient, family, employee, and referral-source information.
- Follow company policies, procedures, and applicable healthcare regulations.
- Perform other related duties as assigned.
Required Qualifications
- Previous healthcare industry experience required.
- Experience working within healthcare, senior care, post-acute care, hospice, home health, assisted living, skilled nursing, physician practices, hospitals, or another related healthcare environment.
- Strong interpersonal and relationship-building skills.
- Professional verbal and written communication skills.
- Ability to communicate effectively with healthcare professionals, administrators, patients, families, caregivers, and community partners.
- Strong organizational, time-management, and follow-up skills.
- Ability to work independently while remaining accountable and connected to the team.
- Ability to manage multiple professional relationships, priorities, referrals, and follow-up needs.
- Understanding of professional boundaries and confidentiality within a healthcare environment.
- Reliable transportation and ability to travel throughout the assigned service area.
- Valid driver’s license and automobile insurance as required for the position.
- Ability to use email, telephone, electronic documentation systems, and basic office technology.
Preferred Qualifications
- Previous hospice or home health experience.
- Healthcare liaison, marketing, outreach, admissions, intake, care coordination, referral development, or business-development experience.
- Existing professional relationships within the local healthcare or senior-care community.
- Experience working with assisted living communities, group homes, skilled nursing facilities, hospitals, physician offices, or senior-care professionals.
Schedule and Work Environment
This position requires community-based outreach and regular travel within the service area. Scheduling may vary based on referral activity, community events, networking opportunities, facility visits, and organizational needs.
Some flexibility may be needed for early morning, evening, or occasional weekend events or activities.
