Job summary

Optimal Care is seeking a Hospice Medical Social Worker to provide compassionate psychosocial care to patients and families navigating end-of-life decisions.

You will conduct assessments, offer crisis intervention, support advance care planning, and collaborate with the interdisciplinary team to integrate emotional, cultural, and practical needs into the plan of care. Some travel within our service area is required.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current state licensure as a LMSW or LCSW.
• Minimum 1 year of experience as a licensed Master's Social Worker.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.
• Willingness to travel to homes, facilities, and community settings.

Responsibilities

• Provide compassionate psychosocial care for end-of-life patients and families.
• Complete psychosocial assessments and crisis interventions.
• Support advance care planning conversations.
• Collaborate with nurses, physicians, and hospice professionals to integrate needs into the plan of care.
