Job summary

Qualifications

• Associate or Bachelor's degree in Nursing required.
• Minimum of one year nursing practice or equivalent experience preferred.
• Hospice, oncology, home health and/or long-term care experience preferred.
• Active RN license in state(s) of employment required.
• Current CPR certification required.

Responsibilities

• Coordinate clinical, psycho-social and spiritual services as indicated by Plan of Care through case management model.
• Collaborate with IDT to meet patient needs for clinical, psycho-social and spiritual interventions.
• Explain hospice services and Medicare benefits to patients and families; obtain informed consent.
• Submit appropriate documentation to facilities at the end of patient visits.
• Provide on-call after-hours support and maintain updated care plans.
