Job summary

The Community Hospice in Albany is seeking a Part-time Hospice Intake & Admissions RN for our Central Intake & Admissions Team. You will be the first contact, guiding patients and families through the transition to hospice care, with a focus on compassion and clear communication.

Responsibilities include coordinating referrals, meeting patients at home or facilities, initiating care plans, and collaborating with the interdisciplinary team to ensure seamless care and a smooth handoff to the

Qualifications

Active NY State RN license required.
Home care experience (1+ year) strongly preferred.
Hospice or end-of-life care experience is a plus.
Valid driver’s license and reliable transportation.
Strong communication with patients, families, and clinical teams.
Clear, concise documentation abilities.
Comfort with computers and electronic health records (EHR).

Responsibilities

Serve as a key liaison between patients, families, providers, and our interdisciplinary team.
Review and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teams.
Meet patients where they are—at home or in care facilities—to complete initial assessments.
Initiate care plans, including ordering medications, equipment, and supportive services.
Partner with social workers, chaplains, aides, therapists, and volunteers to provide holistic care.
Ensure a smooth, supportive handoff to the Hospice Case Manager.
