Job summary

Bluegrass Care Navigators is seeking an Inpatient Social Worker to provide direct psychosocial assessments and counseling for patients and families in the hospice inpatient unit and hospital settings.

Role includes interdisciplinary collaboration, establishing treatment goals, and coordinating care to support emotional and social well-being, under supervision of the Counseling Manager and Executive Director.

Qualifications

• CSWE-accredited social work degree and Kentucky licensure (LSW/CSW/LCSW).
• Minimum of two years social service experience; healthcare setting preferred.
• Experience working within an interdisciplinary team.
• Knowledge of death, dying, and bereavement.
• Understanding of psychosocial and legal aspects affecting terminally ill patients and families.
• Strong written and oral communication; ability to give/receive feedback.
• Time-conscious, organized, self-directed, professional.

Responsibilities

• Provide direct psychosocial assessments and counseling to patients and families.
• Identify needs, establish treatment goals, and implement interventions to alleviate psychosocial challenges.
• Collaborate with the interdisciplinary team to coordinate patient care.
• Support overall care delivery, ensuring patient- and family-centered care in hospice settings.
