Job summary

Community Hospice and Palliative Care seeks an RN Inpatient to provide compassionate nursing care in a 36-hour work week, 7 a.m. to 7 p.m., at UF Health location on 8th Street, Jacksonville, FL. The role emphasizes symptom management, patient- and family-centered planning, and collaboration with an interdisciplinary team.

Requirements include an active Florida RN license and CPR; CHPN is preferred. A $3,500 sign-on bonus is offered to attract experienced candidates who align with hospice

Qualifications

• Active Florida RN license or state of practice
• Current CPR certification
• CHPN preferred or willingness to obtain within a specified time frame

Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans.
• Provide direct nursing care including medication administration and symptom management.
• Educate patients and families about disease progression, hospice care, and end-of-life planning.
• Maintain accurate documentation per policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure patient- and family-centered care.
