Job summary

Ohio’s Hospice, Inc. is seeking compassionate Registered Nurses to join our Hospice Inpatient Unit at The Ohio State University Wexner Medical Center in Columbus, OH. This PRN role supports Day and Night shifts, delivering patient-centered end-of-life care with a multidisciplinary team.

Responsibilities include assessing patient needs, developing care plans, coordinating services, and ensuring high-quality documentation and communication per SBAR standards.

Qualifications

• Current, unrestricted Registered Nurse license in the State of Ohio.
• Two years of acute care nursing experience preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• CPR certification required.
• Proficiency with electronic health records and accurate clinical documentation.

Responsibilities

• Assess the physical, emotional, psychosocial, and spiritual needs of hospice patients and their families.
• Develop, implement, and evaluate individualized, patient-centered plans of care.
• Monitor changes in patient conditions and communicate updates promptly to physicians and the interdisciplinary team.
• Participate in hospice admissions, care conferences, and interdisciplinary team (IDT) meetings.
• Maintain accurate and timely documentation of assessments and interventions in the electronic health record.
• Provide supervision, education, and evaluation of Personal Care Specialists and volunteers.
• Collaborate with other departments to support organizational goals.
