Job summary

Ohio's Hospice, Inc. in Wooster, OH is seeking a PRN Registered Nurse for the Hospice House unit. This role requires providing comprehensive patient care across the nursing process, with occasional float to a sister unit.

The RN will collaborate with the interdisciplinary team, document in the EHR, and participate in CQI and mentoring. Applicants should hold an Ohio RN license, have acute care experience, CPR, and be comfortable driving and meeting the needs of patients and families in a hospice

Qualifications

• Active RN license in the State of Ohio without board actions.
• 2 years acute care nursing experience preferred.
• CHPN certification preferred and available to Ohio's Hospice employees.
• CPR certification.
• Computer skills sufficient to document services and care.
• Valid Driver's License with Safe Driving Record.

Responsibilities

• Plans, coordinates, and delivers patient care using the nursing process, including assessment, planning, intervention, implementation, and evaluation, while collaborating with patients, families, and the interdisciplinary team.
• Performs comprehensive patient and family assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Develops, implements, and updates individualized, patient-centered care plans with measurable goals in collaboration with patients and families.
• Reviews and revises the plan of care, including interventions, medications, and visit frequency, to meet changing needs.
• Assesses changes in patient condition, communicates findings to the physician, and assists in determining appropriate levels of care and continued eligibility for hospice services.
• Participates in hospice admissions, interdisciplinary team meetings, and care conferences to coordinate patient care.
• Documents physician orders accurately in the EHR to support safe, timely, and effective care.
• Supervises, educates, and evaluates Personal Care Specialists, volunteers, and other caregivers to ensure care is delivered per the plan.
• Completes timely documentation of assessments, interventions, and changes in patient status at the point of care in the EHR.
• Provides clear and timely handoff communication (SBAR) to ensure safe transitions.
• Supports organizational needs to ensure high-quality care for all Ohio's Hospice patients.
• Participates in CQI initiatives to enhance patient outcomes and organizational performance.
• Serves as a preceptor and mentor for newly hired clinical staff.
