Job summary

Ohio's Hospice in Wooster, OH is seeking an LPN Hospice House - PRN (registered nurse scope) to support inpatient unit care. The primary site is in Wooster with occasional float to the sister unit in New Philadelphia.

Qualifications

• Active RN license in the State of Ohio without board actions.
• Two years acute care nursing experience preferred.
• CPR certification and strong computer/documentation skills.
• Ability to drive during daytime or night time and inclement weather.

Responsibilities

• Plan, coordinate, and deliver patient care using the nursing process with interdisciplinary team.
• Perform comprehensive patient and family assessments addressing physical, emotional, psychosocial, and spiritual needs.
• Develop and update individualized, patient-centered care plans with measurable goals.
• Review and revise the plan of care, medications, and visit frequency as needs change.
• Assess changes in condition, communicate with physician, and determine care levels.
• Participate in admissions, IDT meetings, and care conferences to coordinate care.
• Document orders accurately in EHR to support safe, timely care.
• Supervise and educate Personal Care Specialists and volunteers; ensure plan adherence.
• Complete timely documentation of assessments and changes in patient status in EHR.
• Provide clear handoffs using SBAR for safe transitions.
• Assist other departments to support high-quality care; participate in CQI initiatives.
• Serve as preceptor/mentor for new staff as assigned.

Education

Active RN license (Ohio)

BSN preferred

CPR certification

Excellent computer skills for EHR documentation

Valid driver's license with safe driving record
