Job summary

SSM Health at Home, Wentzville, MO, is seeking a Hospice Registered Nurse to coordinate and perform interdisciplinary plans of care in patients' homes and community settings. Opportunity includes travel to patient homes, CB RFs, assisted living locations, and hospitals to assess needs and provide skilled nursing care under physician direction.

Schedule is typically M-F 11a–7:30p with on-call as needed; requires valid RN license, BLS, and ability to travel within Missouri counties.

Qualifications

• RN licensed in MO/IL/OK/WI or compact state
• BLS Health Care Provider certification
• Valid driver’s license and auto insurance
• Ability to travel to patient homes and communities
• No prior experience required

Responsibilities

• Assess and coordinate patient discharge planning with the healthcare team
• Perform point of care testing per policies
• Provide updates to physician and revise plan of care as needed
• Coordinate with multi-disciplinary teams and outside agencies to meet goals
• Complete admission paperwork, comprehensive medical assessments, and timely documentation
