You’ll assess psychosocial, financial, and environmental needs, provide crisis intervention, and contribute to care planning with the interdisciplinary team.

Qualifications

• Master’s degree from an accredited School of Social Work.

Responsibilities

• Conducts clinical assessments identifying psychosocial, financial, environmental, and community needs.
• Provides crisis intervention and supportive counseling to patients and families.
• Contributes to care planning and coordinates with the interdisciplinary team.
• Facilitates discharge planning and supports referrals as needed.
