Job summary

Mission Healthcare in California seeks a qualified Social Worker to join our home health and hospice teams. You will assess psychosocial needs, collaborate with physicians, nurses and chaplains to tailor care plans, and support families with advance care planning and grief counseling.

The role requires an MSW and at least one year of healthcare experience, with hospice or home health care preferred. Valid driver’s license and transportation are expected, with best-fit hours M-F 8am-5pm.

Qualifications

• Minimum of one year of healthcare experience.

Responsibilities

• Assess the psychosocial status of patients and families.
• Collaborate with the interdisciplinary team to develop care plans.
• Assist with advance care planning and end-of-life discussions.
• Conduct social evaluations and plan interventions.
• Maintain clinical records for referred patients.
• Provide information and referrals for practical and environmental needs.
• Offer individual, family, and group counseling.
• Support families with caregiving challenges.
• Liaise with community agencies for financial or legal support.
