Job summary

Must have MSW; CA LCSW preferred; CPR certification; 1 year hospice/healthcare experience; valid CA driver’s license; ability to lift up to 50 pounds; strong written and verbal communication.

Qualifications

• Master of Social Work from a CSWE-accredited program
• Minimum 1 year of experience in health care or hospice
• Current CPR certification
• Valid California driver’s license with insured vehicle
• Ability to lift up to 50 pounds

Responsibilities

• Assess the psychosocial status of patients and families/caregivers and communicate findings to the team
• Counsel patients and families, provide crisis intervention when needed
• Educate interdisciplinary group on cultural needs and preferences
• Maintain clinical records and prepare progress notes
• Assist in preparation of advanced directives and referrals
• Participate in care planning and team meetings
• Maintain liaison with community agencies and resources
• Support quality improvement initiatives
