By The Bay Health in California is seeking a Hospice Team Nurse (RN) to provide coverage for Nurse Case Managers, coordinating care with the transdisciplinary team. Schedule: 3 days part-time, 8:30am - 5:00pm, location Sonoma County.

The role emphasizes nursing process, patient/family assessment, care planning, and service coordination in home-based hospice and palliative care. California RN license and home health/hospice experience preferred; CDPH vaccine requirements apply.

Qualifications

• One year experience as a professional nurse in the past three years or; Bachelor’s degree in Nursing from an accredited School of Nursing
• Experience in home health and/or hospice setting preferred
• Current license as a registered professional nurse in the State of California

Responsibilities

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
• Coordinate all patient/family services and prioritization of needs with the members of the transdisciplinary team
• Use a case management approach and refer to other services as needed
• Coordinate the implementation of the plan of care and insure care coordination and case management for each patient

We are seeking a Hospice Team Nurse (RN) who will provide coverage for Nurse Case Managers to cover patient/staffing needs in coordination with Team Managers.

Schedule: 3 days part-time; 8:30am - 5:00pm

Location: Sonoma County

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

$58.30 - $72.95 hourly
