Job summary

Health Link Home Health and Hospice in the San Francisco Bay Area is seeking a compassionate Hospice Registered Nurse (RN) to provide skilled, patient-centered care in patients' homes. You will assess needs, manage symptoms, educate families, and coordinate with an interdisciplinary team led by the Hospice Clinical Director to ensure comfort and dignity at the end of life.

Qualifications

• Must hold an active California RN license.
• Minimum 1 year nursing experience.
• Hospice or home health experience preferred.
• Ability to work independently in the field.
• Strong clinical assessment, communication, and organizational skills.
• Comfort with electronic medical records and mobile tech.

Responsibilities

• Provide skilled nursing care in accordance with each patient's individualized hospice plan of care.
• Perform comprehensive nursing assessments and evaluate patients' physical, emotional, and comfort needs.
• Provide symptom and pain management interventions to promote comfort and quality of life.
• Administer medications and treatments and evaluate patient response.
• Educate and support patients, families, and caregivers on medications and end-of-life care.
• Communicate changes in patient condition to the Hospice Clinical Director and interdisciplinary team.
• Coordinate care with physicians, hospice aides, social workers, chaplains, facilities, pharmacies, and other team members.
• Participate in interdisciplinary team collaboration and care planning.
• Complete accurate clinical documentation using Kinnser/WellSky on company devices.
• Maintain compliance with hospice regulations and safety requirements.
