Job Responsibilities

The Certified Nursing Assistant/Home Health Aide (CNA/HHA) works under the direction of a Case Manager/DON, providing resident care in accordance with assessments, care plans, and nursing management directives. The role involves coordinating care with licensed staff, adhering to company policies, procedures, and safety guidelines.

Qualifications and Skills

• Strong commitment to improving quality of life and partnering with community organizations
• Ability to work collaboratively with nurses, social workers, chaplains, volunteers, and leadership
• Aspiring to attain higher certifications and education
• Ability to work closely with patients on a one-on-one basis, with support from a care team
• Ability to work independently on a daily basis
• Willingness to drive with reliable transportation, valid driver’s license, and auto insurance
• Certified Nursing Assistant (CNA) certification in the state(s) of practice
• Home Health Aide (HHA) certification in the state(s) of practice
