Legacy Lifecare Hospice and VNA is seeking a Hospice Home Health Aide (CNA) in Massachusetts. This PRN role offers $20.00-$25.00/hour, Monday–Friday, 8:00AM–4:00PM, based on experience. You will provide hands-on care, monitor changes, and support families in end-of-life settings.

Requirements include current MA CNA certification, at least 1 year of CNA experience, a valid driver's license, reliable transportation, and excellent English communication. Travel to patient homes is expected.

Qualifications

• Current, active MA CNA certification is required.
• Minimum 1 year of CNA experience.
• Valid driver's license is required.
• Reliable transportation and ability to travel to patient homes.
• Prior home-care, hospice, rehab, or long-term care experience preferred.
• Ability to communicate fluently in English.

Responsibilities

• Provide compassionate, hands-on personal care to hospice patients.
• Monitor and report changes in patients’ condition to the hospice team.
• Maintain a safe, clean home environment for patients and families.
• Offer emotional support and companionship while respecting patient dignity.

• Provide compassionate, hands‑on personal care to hospice patients, including bathing, grooming, dressing, toileting, and mobility assistance.
• Monitor and report changes in patients' physical condition, comfort, behavior, and needs to the hospice nurse or care team.

• Offer emotional support and companionship while respecting patient dignity, preferences, and end-of-life care goals.

Qualifications for Success:

• Current, active MA CNA certification
• Minimum 1-year CNA experience
• Must have a valid driver's license
• Must have reliable transportation and the ability to travel to patient homes or hospice facilities.
• Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred

• Ability to communicate fluently in English – both verbally and written
