Job summary

Legacy Lifecare Hospice and VNA is seeking a Hospice Home Health Aide (CNA) to provide compassionate, hands-on care in patients’ homes. Responsibilities include bathing, grooming, mobility assistance, and reporting changes to the hospice team.

The role requires active MA CNA certification, at least 1 year of CNA experience, and a valid driver’s license with reliable transportation. Travel to patient homes or hospice facilities is expected.

Qualifications

Responsibilities

• Provide compassionate, hands-on personal care to hospice patients (bathing, grooming, dressing, toileting, mobility).
• Monitor and report changes in patients’ condition to the hospice nurse or care team.
• Assist with maintaining a safe, clean, and comfortable home environment for patients and families.
• Offer emotional support and companionship while respecting patient dignity and end-of-life goals.

Hospice Home Health Aide - CNA | Legacy Lifecare Hospice and VNA
$20.00 - $25.00/hour, based on experience
PRN | Monday – Friday | 8:00AM – 4:00PM

• Monitor and report changes in patients' physical condition, comfort, behavior, and needs to the hospice nurse or care team.
• Assist with maintaining a safe, clean, and comfortable home environment for patients and their families.
• Offer emotional support and companionship while respecting patient dignity, preferences, and end-of-life care goals.

Qualifications for Success:

• Current, active MA CNA certification
• Minimum 1-year CNA experience
• Must have a valid driver's license
• Must have reliable transportation and the ability to travel to patient homes or hospice facilities.
• Prior experience in home-care, hospice, rehabilitation, or long-term care setting preferred

• Ability to communicate fluently in English – both verbally and written
